Fluent Forms Password reset option

Hi all,

Hoping someone can assist, I wish to add a password reset field on woocommerce my account page for a client who wishes to amend there password while in their account page if thats at all possible please.
Someone posted this snippet that can be used however i am not sure how to edit this snippet in order for it to work.

Im hoping someone can assist or can this even be added to an update since i see a few people having the same issue.
