Form not displaying dynamic data

I want to create a client info update form.

I have duplicated the basic and additional custom customer fields into a fluent form.

I have set the default values to the CRM info however only the first, last and email are displayed. I used the get code for the phone, Date, and custom fields yet the info doesn't appear when the form is loaded.

I have included screenshots of the form front end, form fields with get code, CRM contact fields with completed info, and the form integration fields.

issue:

1 not all CRM contact details are dynamically populated in form front end.

How do I resolve this any advice will be useful
