Custom list based on page author?

I want to create a subscription form to add to a detail page of a CPT. I would like to add to a list based on the author of the CPT. Is this possible?

I have a CPT of books. I want to embed a Fluent Form on each Book page that would subscribe to the authors list.

Ideas?
