Editing the Post -> Removes image?

Have you noticed that sometimes when you edit the post which has an image, the image disappears? For example, I changed the title, and I lost the image attachment. I had to upload the image again. Then on the next try, it didn't disappear. But after that, the image disappeared again. Can you check this? Thanks.
