Changing profile picture error

I tried to change my existing user profile picture with the same picture name (JPEG - 600x600) uploaded from my computer and I face the error.

Also I am not sure why previously the User Profile picture was not visible in the top right corner (dropdown) but it was visible on the User Profile - see attachment.
